JOB SUMMARY
The post holder will work as an activities co-ordinator across the acute wards and will deliver a range of practical activites, within the hospital setting. Some of these activites will take place in the day as well as evenings and at weekends. The activites will reflect the needs identified on the acute wards. Patients admited to the wards are in the acute stage of illness and, as needs fluctuate rapidly, the nature of the service is dynamic.
The post holder will be required to work with a nurse in the provision of some activites so that the nurse can write a report about the therapeutic value of the group for recording on RIO as part of the Care Plan.
Patients on the wards have a wide range of needs and the post holder will be required to be flexible and creative in addressing these.
Duties:
Clinical responsibilities
- To plan and carry out a range of practical activites in conjunction with the ward staff and to input into the evaluation and review of these activites for patients on the acute ward in groupwork.
- To liaise with other members of the multi disciplinary team regarding the functional and mental ability of individual patients, as observed in a range of practical activites.
- To work in collaboration with others to modify aspects of the enviroment in order to empower patients and facilitate optimal functional performance in practical activites. In conjunction with ward staff, to adapt activites and environment according to any special needs, eg physical limitations, expressive and receptive language difficulties. To maintain a flexible approach at all times.
- To ensure all relevant information regarding patient care is communicated, verbally, to the appropriate people providing a service to the acute wards.
- To keep up to date with facilities available in the community, to facilitate access to the same for patients as appropriate and to liaise with staff working in these facilities.
- To take a lead role in the drawing up of a programme of practical activites, following consultation with ward staff and EMU. To provide information in the evaluation of this programme.
- To ensure correct and safe handling of equipment and materials, and clear instruction on the implementation of activities at all times.
- To work in collaboration with others to improve the environment and create as therapeutic environment as is possible for practical activites to take place.
- To ensure that practical activites are carried out with a level of expertise. To demonstrate an awareness of and compliance with any models of care in use and approaches to precautions taken as a result of risk assessment.
- To communicate clearly and accurately infomation about patients and handovers. This may include complex and sensitive information which must remain confidential amongst the team.
- To ensure there is effective two way communication with ward staff before and after sessions take place.
- To work flexibly and change the adapt activites, in response to changing needs.
- To be aware of individuals' risk assessments and to take these into account in every aspect of intervention. To ensure a consistent approah with other team members in relation to this and othe aspects of the individuals' care plan.
- To carry out delegated tasks to support aspects of individuals' care plan which relate to practical activites.
